I work in a medical office and we are changing location and phone numbers I need to send a letter to all the health that we deal with a letter stating our changes.

Our tax identification number will remain the same just our physical address and our phone number.

Any suggestions on a professional letter to write with the changes?

    a simple letter is stating you are moving locations and include your old address and phone number and the new ones. make sure to include your tin, so they can update their records correctly.

    the simpler the better.
